Ministry of Health and Welfare (MOHW) and MSIA Exchange: Building Global Smart Healthcare with FHIR and '3-3-3 Strategy'

The Ministry of Health and Welfare Information Department held an exchange with Emma Hossack, CEO of MSIA, presenting achievements in digital health interoperability. Ms. Hossack shared Australia's 13-year journey from CDA to FHIR, highlighting the challenges of early data formats. Taiwan showcased its independently developed 'FHIR Box' architecture. This is presented as a crucial architectural layer—a 'data middle platform' that enables data transformation, validation, and exchange without requiring the replacement of existing Hospital Information Systems (HIS). This makes Taiwan one of the world's first nations to build a nationwide interoperability system using open standards without replacing its core HIS. Furthermore, the MOHW detailed its '3-3-3 Strategy,' which integrates three major international standards: LOINC, RxNorm, and SNOMED CT. In terms of technical achievements, Taiwan has achieved national LOINC coding coverage (over 5,050 items) and is pioneering the use of LLMs for SNOMED CT clinical coding. The establishment of robust governance mechanisms, including AI centers and standardized data flows, underscores a comprehensive approach to smart healthcare that aligns with global best practices.
