Digitalization of Community Care: Discussing the Construction of a Shared Information Platform Using FHIR Standards

A 'FHIR Health Data Analysis and Decision Expert Consultation Meeting' was held as part of the East District Comprehensive Care Plan. Medical teams from hospitals in the area recognized the necessity of standardizing and linking data to achieve comprehensive community care (whole person care). Furthermore, the use of AI for automated recommendation and screening of data was proposed, expecting to 'standardize' and 'accelerate' the data processing process. Ultimately, they agreed on building a regional collaborative platform, planning future efforts focused on talent training, incentive systems, and establishing a comprehensive data structure that includes cross-institutional long-term care data and Social Determinants of Health (SDOH). This initiative positions 2026 as the 'Year One of Transformation' for the East District Comprehensive Care Plan, driving the reform of the regional medical system toward preventive medicine and whole-person health.
